
Care Coordinator Team
Care coordinators proactively identify and work with patients, including the vulnerable, frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.
Their role helps to ensure that your health and care planning is timely, efficient, and patient-centred.
Aspects of their work include the coordination of the patient journey through primary care and secondary care. This is achieved by bringing together all the information about your identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to you.
We have a team of six Care Coordinators. Some of them work across all of our member practices focusing on a dedicated role such as Safeguarding or Care Homes and patients under the MDT team. Others are practice specific and undertake work solely for the patients of the practice in which they are based.
They work closely with other members of the Primary Care Team such as our Social Prescribers and Health & Wellbeing coaches to ensure you have a personalised package of care that is shaped to meet your individual needs as much as possible.

Hayley Sumner
Safeguarding Lead
Hayley is a key part of the Care Coordination team and covers all three practices in the PCN managing all things safeguarding.
Prior to joining the PCN Hayley worked as a Senior Social Worker in a Family Safeguarding Team. Hayley has substantial frontline Social Work experience from Early Help through to Assessment and Intervention, Child in Need and Child Protection Work, Court Work with experience in both Public and Private Proceedings. Hayley has strong analysis and assessment skills and she can build and sustain positive working relationships with both families and professionals.
The Safeguarding Lead role is to oversee and manage the quality of Safeguarding at the practices within the PCN. Hayley provides support to all staff and ensure timely information exchange between relevant safeguarding partners within the legal framework. Hayley supports the strategic direction across the organisation.

Emma Cope
Care Home & PAC MDT Lead
Emma joined Kingston PCN in March 2024 & works across all three practice’s assisting in the care of vulnerable patients’ and those patients’ in the local care homes, specifically Kingston Care Home & Anne Residential.
Emma has many years of previous experience working in the care sector.
Emma works closely with the Practice’s clinical leads, visiting the care homes on a weekly basis, and attending the MDT meetings to discuss vulnerable patients with other members of the MDT from both primary, secondary and community care.
Emma feeds back on the outcomes of the MDT and works alongside practices to ensure that appropriate measures are put in place to support our most high risk patients.

Amy-Louise Bowden
Care Coordinator - Fairhill
Prior to joining the PCN Amy-Louise worked as part of the administrative team at Fairhill Medical Practice. She joined the PCN in February 2024 as a Care Coordinator. Amy-Louise’s role includes coordinating referrals and appointments for the Community Paediatric Hub and is responsible for keeping record of the children who require vaccinations.

Susan Murray
Digital Care Coordinator - Fairhill
Susan is a Digital Care Coordinator monitoring and directing incoming emails to the appropriate team to be dealt with, as well as managing reception, dealing with queries and booking appointments, and processing new patient registrations at the Fairhill Medical practice University branch.
Prior to becoming a Care coordinator Susan was previously a System Administrator managing IT and the computer network within a secondary school. Susan is highly proficient in learning and using various IT skills.

Danielle Parker
Care Coordinator - Fairhill
Danielle Parker joined Kingston PCN in July 2024 as a Care Coordinator for Fairhill Medical Practice. Danielle’s role includes daily management of Docman workflow, where she manages the clinical letters and emails the Practice receive and workflow these to the relevant GP’s to action. Danielle also assists the practice by booking patients for their Diabetic review, sending reminder texts for patients overdue their Bowel Cancer and Breast Cancer Screening and reviewing CQC Markers for patients registered as Carers, this involves helping to ensure that Carers are able to access the help and support within the local community.

Rupa Kandala
Care Coordinator - St Albans
Rupa joined Kingston PCN in January 2025 as a Care Coordinator for St Albans Medical Centre. Rupa’s role includes coordinating patient's care by monitoring, inviting and booking appointments for patients at St Albans Medical Centre. Rupa has two years’ experience working as a receptionist in Primary care prior to joining the PCN.
